FINANCIAL SECRETARY REMITTANCE FORM Guidelines

This form is used when any money is sent to the LWML SD District. This includes Mite Offerings, Lutheran Woman's Quarterly Subscriptions, HiLites Subscriptions, Stamps, Special Gifts Fund, Memorial Mites, and a spot for other donations.

When sending your group’s subscription payments, please include the number of subscriptions ordered on this form. If paying for the current year’s subscription, include the year on the form. For example: #10 x $5=$50 (2014). Many groups pay for subscriptions at the end of the year and it is difficult to tell if the payment is for the current year or upcoming year.

This form has had a few changes over the years. Please note that all money is sent to the Financial Secretary and no longer the Treasurer and that Quarterly and HiLites Subscription rates have increased over the years. Please, destroy all old forms and feel free to make as many copies of this form as are needed.

This form is not used for money that goes to each zone, including zone pro-rates/dues. Those are sent to the Zone Treasurer.
